Below are several RTA Forms for your convenience - please download and complete a copy or contact us if you would like us to provide one for you:
- Form 1a - Entry Condition Report - General (PDF)
For use in general tenancies (i.e. houses, units). - Form 6 - Change of Shared Bond Arrangement (PDF)
When there is a change to the people contributing to the bond. - Form 11 - Notice to Remedy Breach (PDF)
Use if one party has broken the tenancy agreement. - Form 13 - Notice of Intention to Leave (PDF)
Used by the Tenant to notify the lessor they wish to vacate the premises. - Form 14a - Exit Condition Report - General (PDF)
Completed by the Tenant. Shows the condition of the premises when the Tenant leaves. - Form 16 - Dispute Resolution Request (PDF)
Used by either Agent/Lessor or Tenant to seek the RTA's assistance to resolve a dispute.
